Membership Form

RWIZI-NETWORK MEMBERSHIP CRITERIA

You will be required to fill a RWIZI-NET membership form and attach the following documents:

  • Certified copy of Permit to Operate as NGO/Certificate of Registration issued by NGO Bureau.
  • Certified copy of Certificate of Registration and Incorporation issued by any relevant authority.
  • Profile of Programme/Activities being undertaken
  • Brochure
  • Sketch Map of Location of Organization.
  • Organizational Organogram

    • An assessment of the fully filled membership form will be carried out and a communication made in approval or otherwise.

    An Annual Membership Subscription Fee of UGx: 50,000/= ONLY shall be payable at the RWIZI-NET bank account and receipt issued should the membership request be approved.
